Overview

Provide case management, psychosocial support, and awareness-raising activities for people with specific needs. The role involves direct support, referrals, and ensuring the safety and inclusion of affected populations.


Key Responsibilities
  • Provide specialized case management including psychosocial support to people at heightened risk.
  • Support identification of people at heightened risk through home visits and coordination.
  • Provide age- and disability-appropriate individual counseling and psychosocial support.
  • Ensure timely filing and storage of case management data, maintaining confidentiality.
  • Work with other teams and service providers to ensure access to relevant services.
  • Prepare and submit weekly and monthly work plans.
  • Support the implementation of monitoring and evaluation tools.
  • Participate in supervisory meetings.
  • Develop and maintain working relationships with stakeholders.
  • Contribute to service mapping.
  • Attend case conferences when needed.
  • Raise gaps in services and referral protocols.
  • Immediately flag safety concerns.
Required Experience

Minimum 2 years of experience in delivering specialized case management or counseling to PWSN required. Previous experience in international organizations and humanitarian settings is a plus.

Qualifications

Bachelor's in social work, Counseling, Psychology or other related field.
