Overview

The role of Community Health Worker involves identifying and supporting vulnerable individuals, conducting health screenings, and promoting health practices within targeted communities.


Key Responsibilities
  • Conduct home visits to identify pregnant women, nursing mothers, and children under five.
  • Perform community screening for acute malnutrition in children using MUAC and edema.
  • Refer malnourished children and at-risk pregnant women to health facilities.
  • Follow up on referred children for treatment and recovery.
  • Train and support mother leaders and community relays on health, nutrition, and hygiene.
  • Conduct community awareness sessions on breastfeeding, infant feeding, hygiene, and disease prevention.
  • Promote essential family practices for maternal, neonatal, and child health.
  • Participate in vaccination campaigns, vitamin A supplementation, and deworming.
  • Identify and report priority diseases, epidemics, or health emergencies.
  • Collect, compile, and transmit health and nutrition data.
  • Participate in community meetings and coordination with health structures.
  • Support community accountability mechanisms and complaint management.
Required Experience

Minimum of 2 to 3 years of experience in community health or nutrition activities. Experience in MUAC malnutrition screening. Experience in community mobilization and awareness. Experience using smartphones and digital data collection tools.

Qualifications

NS2 level or completed secondary education. Training in community health, nutrition, social work, or a related field. Certificate in community health, nutrition, health promotion, or a similar field.
