Overview
The NCD HeartCare Outreach and Data Focal Person supports the Ministry of Health NCD team in planning and coordinating NCD outreach activities, analyzing HeartCare data, and strengthening data quality for programme management and decision-making.
Key Responsibilities
- Support NCD outreach planning and coordination, including preparing plans, liaising with teams, and tracking completion.
- Analyze and present HeartCare data for programme action, including generating summaries, charts, and reports.
- Support identification and follow-up of high-risk patients by generating lists and tracking follow-up.
- Strengthen HeartCare data quality through routine checks and identifying areas needing support.
- Support HeartCare user adoption and provide basic user support.
- Support monitoring, reporting, and programme documentation, including submitting regular reports and documenting lessons learned.
- Coordinate with Ministry of Health, WHO, and partners on NCD activities and data management.
Required Experience
At least five years of demonstrated experience supporting health programme coordination, outreach activities, public health services, health facility support, or community-based health activities. Demonstrated experience working with routine health data, programme data, monitoring tools, reporting systems, or health information systems.
Qualifications
University degree in public health, health information systems, health sciences, nursing, data management, statistics, information systems, programme management, or related discipline.
